At the same time, there remains much to be done to build services that fit the unique profile, needs, and preferences of persons living with YOD. In this piece, Emily Ong, a dementia self-advocate who has YOD, highlights this issue in greater depth, calling for the right support for young-onset dementia.

There is a pressing need for timely, appropriate post-diagnostic support and interventions for persons with young-onset dementia.

While there has been a growing focus on developing community awareness of dementia in Singapore, the special needs of younger people living with dementia and their families should be properly addressed. Many existing post-diagnostic services and support systems are developed with the elderly in mind, since dementia is more prevalent in this age group. This results in services and support that may not adequately meet the specific needs of those affected by young-onset dementia.

Services and intervention have often been focused on cognitive stimulation therapy. Yet, we know that the impact of a diagnosis of dementia can cause significant family distress and social isolation, and services should address these issues too. Younger persons living with dementia and their families have a greater need for emotional, socio-psychological and relationship support, regardless of their dementia stages upon diagnosis. The sense of grief over the ‘loss of person within’ and multiple other losses can have major effects on persons with dementia and their life together as couples and family units.

It’s important to realise that generic dementia services, often designed for older cohorts, are not suitable if the nation wants to help younger persons with dementia, who are often still physically fit, maintain an active life and stay independent for as long as possible. As a person living with dementia, I would say we need post-diagnostic support and interventions that support us in engaging socially, enable us to be involved meaningfully, and empower us in a dignified manner. A diagnosis of dementia does not make an individual a lesser being. We still have the same needs, and want to continue feeling like valuable, productive, and contributing members of our family, community, and society.

An increasing number of people are diagnosed with young-onset dementia each year, and it is imperative that we consider these special needs when planning future dementia policies and implementing post-diagnostic support and interventions. Policy makers, health, and social care providers should proactively consult and engage younger persons living with dementia and their families to ensure that what is implemented is appropriate. This will leave those with young-onset dementia, and their families, better equipped to deal with changes in abilities, relationships, and roles that may arise as the condition progresses. We need more than a ‘one-off’ kind of support- ongoing guidance and support is necessary to enable them to live well and meaningfully despite dementia.

Emily Ong shares on her a journey as a dementia advocate, and how she has stepped up her advocacy efforts at the regional and global levels.

I joined Dementia Alliance International (DAI) in August 2019 after I heard about Kate Swaffer from my discussion with Ruth Wong, ADA’s Voices for Hope Programme Manager. It is inspiring and motivating to know a global organisation meant for and run by people living with dementia. I would say that it is the best testimony of the ability and capacity of people living with dementia.

My first article contribution for the DAI blog was in conjunction with World Alzheimer’s Month #WAM2019. “#Hello, My name is Emily Ong” was published on Day 9 of Dementia Awareness Month/ World Alzheimer’s Month. It was a heroic act and marked the beginning of the first page of the first chapter of my journey as a dementia advocate.

June 2017 (the exact date I could not remember already) I had the scariest experience of my life when I asked myself – “What do I need to make French toast?” – when it has always been my family’s usual breakfast. All that I remembered was bread, and that the equipment I needed was a skillet.

Since then, my advocacy journey has been sailing wide and far with the support of Kate Swaffer, the co-founder, past Chair, and current CEO of DIA, who has been mentoring me all this time. To me, advocacy is using your story and lived experiences as a person living with an invisible disability that is highly stigmatised, discriminated against, and heavily patronised, to call for social change and action to make life better for people living with dementia.

I gave myself two years to grow as an advocate and sharpen my advocacy skills and knowledge. I started with my Facebook Page – Living with mild cognitive impairment & YOD, where I write about what life is like living with dementia, coping strategies, and share information on dementia. When ADA invited me to co-facilitate their step-up programme, Voices for Hope, I took it up and have since co-designed the content and co-facilitated four cohorts. To me, having a dementia advocate involved in the planning and facilitation of a programme is critical to achieving the programme’s goal of encouraging people living with and affected by dementia to step up. It is also about embracing an inclusive work culture within ADA to ‘walk the talk’ of including persons living with dementia in the workforce.

Over time with the support of Kate Swaffer and DY Surhaya, the Regional Director of Alzheimer’s Disease International (ADI) Asia Pacific Region, I began to step up my advocacy at the regional and global levels. I am involved in the World Health Organization’s Global Dementia Observatory Knowledge Exchange Focus Group, ADI Accreditation Global Review Panel, and was recently invited to be in the Scientific Programme Committee for the ADI 2022 Conference. Moreover, my exposure at the ADI 2020 Conference as a keynote speaker and delivering the opening speech on HOPE, has greatly shaped my advocacy path. I have become more knowledgeable about the United Nations Convention on the Rights of Persons with Disabilities (UNCRPD), accessibility, and environmental design’s impact on quality of life.

At the launch of the World Alzheimer Report 2020 on 21 September 2020, I was inspired by Kate Swaffer’s call for action from the rhetoric of dementia-enabling design to the implementation of environmental design. The opportunity came when Associate Professor Habib Chaudhury, a visiting professor at the National University of Singapore, attended one of the sessions of Voices for Hope. He was interested in sharing his expertise in environmental design in building dementia-enabling environments.

With his support, I started a small global group – Environment & Dementia: Special Interest Group (DE-SiG) – in May where professionals from multidisciplinary fields and people living with dementia come together to provide expert advice and inputs on working towards a dementia-enabling design in the community to improve the quality of life for everyone, not limited to people living with dementia. There is a need to look into how the environment, including the natural, built, and communal environment, can be designed to provide tranquility; minimise impairments impact, activity limitation, and participation restrictions from dementia; and can be intentionally created to reflect the shared values of a community.

With all these involvements and working with others at the international level, I am confident and ready to take on the role as a member of DAI’s Board of Directors. DAI is a global group of, by and for people living with dementia, providing a unified voice of strength, support, and advocacy on the rights and autonomy of people living with dementia. It started with 8 people from 3 countries (the US, Canada, and Australia), and now DAI represents members all over the world. My hope and plan as a member of the Board of Directors are two-fold:

To provide an Asian voice from the Asia Pacific region in DAI; to give the global organisation a better representation of the diverse cultural communities it represents. The needs and voices of the non-English speaking dementia communities are underrepresented at the global level. The DE-SiG initiative is an example of how DAI hopes to bring in more experts, particularly from the Asia Pacific region to discuss and find ways to create an enabling environment that is culturally appropriate and needs sensitive.

To grow the presence of DAI in the Asia Pacific region through a local cultural approach so that the support and resources of DAI can benefit a wider and more diverse range of global communities. The DAI SG peer-to-peer support (La Kopi, Lim Teh) is one such initiative where support is adapted to the local culture without compromising on the quality of the support rendered by DAI.

At DAI, we believe that by working together, we will identify concrete action for implementation with the international community, and in the process, ensure our human rights are being fully met. At the end of the day, we want people to know and recognise the importance of embracing “Nothing about us, without us” in initiatives intended for people living with dementia.

As dementia becomes more prevalent in Singapore, ADA has been steadfast in its belief, supporting those impacted by the condition. With the launch of dementia-inclusive initiatives, ADA hopes to enable seniors and persons living with dementia to continue to age-in-place, live and thrive in their familiar areas. Our Wayfinding Project at Kebun Baru is one such initiative, where we have been actively walking the ground since 2018, increasing dementia awareness among its grassroots members and residents.

In March 2019, the Wayfinding Project was initiated after a dialogue with Mr Henry Kwek, Member of Parliament for Kebun Baru Single Member Constituency. The focus group discussion was chaired by ADA and persons with dementia.

Across Singapore, there are 14 dementia-friendly communities, which are localised ground-up efforts aimed at building a caring and inclusive community supportive of those impacted by dementia. An initiative by the Ministry of Health and the Agency for Integrated Care (AIC), these efforts were launched in collaboration with community partners.

Other wayfinding projects led by AIC have also been initiated at Nee Soon and Yio Chu Kang, involving project owners and design experts. These included colour-zoning of residential areas, icons and murals so that these housing blocks and commonly accessed routes are more recognisable.

At Kebun Baru, the Wayfinding Project led by ADA involved the collaboration of the grassroots members to identify housing estates with high proportions of seniors and involved persons living with dementia. As of April 2021, ADA has completed 37 murals across 10 blocks along Ang Mo Kio Avenue 3 and 4.

While there are different initiatives to build dementia-friendly communities, one unique element of the Wayfinding Project at Kebun Baru is the participation of people with dementia in discussions and the execution. By doing so, not only does it cement our belief in practicing inclusiveness in everything we do, it empowers persons with dementia and go a long way in changing the perception of others towards persons with dementia, seeing them as able contributors.

We spoke to Ms Emily Ong and Mr Anjang Rosli, two of ADA’s volunteers who are living with young-onset dementia, and they shared their perspectives and experiences as being part of the project team that spearheaded the iconic murals in Kebun Baru.

1. Tell us how you got involved in the Wayfinding Project at Kebun Baru?

Emily: The opportunity to be part of this project came about after our dialogue with Mr Henry Kwek, MP of Kebun Baru Single Member Constituency. This dialogue was organised as part of our Voices for Hope programme where we, persons living with dementia and our care partners, are empowered on a self-advocacy journey.

Prior to this dialogue with Mr Henry Kwek, I was so excited that I asked my hubby to take me to a Kebun Baru town centre to do a ground sensing of the place and experience navigating through the town as a person living with dementia. During the dialogue session, we discussed different ways the community-at-large can better support people living with dementia who are staying in their local neighbourhood.

Anjang: I was invited by ADA to participate in the mural wayfinding project. I was happy and excited as I was lost in my own neighbourhood (in Woodlands) before because of how similar all HDB void decks are. The lift landings and lobbies all look so similar that we can lose our way quite easily.

2. What was the whole process like for you?

Emily: The whole process was empowering because I got to work with other individuals who were part of the project team. I was able to readily share my views, and my voice was valued and listened to by others in the project. I would say that our involvement – Anjang and myself – and the overall collaboration were a big asset to the project. The outcome was a fusion of perspectives from everyone involved – people with the lived experience of how the provision of murals will assist in wayfinding, care professionals from ADA, and the artists, as well as the Kebun Baru grassroots community leaders.

Ultimately, it is not just about having visual cues. But more importantly, how these visual cues can be “enablers” to support someone living with dementia to remember and identify his or her block and confidently navigate their way back home.

Anjang: The process was very empowering for me. It was really a team effort. Happy to also be able to work with other persons living with dementia. It felt good because I know that whatever is put in place will truly be helpful to people with dementia. We were involved in exploring where to place the murals, the exact walls to paint that will help persons with dementia recognise their blocks as they walked pass. It was fun, and a huge sense of achievement after completion.

3. Which mural is your favourite, and why?

Emily: The ‘ang ku kueh’ mural has a special meaning for me because it is the favourite kueh of my late grandma and me. This affective memory is crucial for persons living with dementia because it is the strongest form of memory for any individual. Hence, tapping on the affective memory is the cornerstone of the Wayfinding Project at Kebun Baru.

Anjang: My favourite is the rabbit sweet. It really brings back fond memories. It reminds me of my younger days when I used to have to walk 3km back home from school every day. The rabbit sweet was my daily companion during those long walks.

4. We are aware of the Nee Soon HDB blocks that are colour-coded to help elderly with dementia move around, and we are also aware of this wayfinding project in Kebun Baru that are also intended to aid in wayfinding. What are your thoughts on the two different approaches?

Emily: Human navigation consists of two main processes: wayfinding (deciding where to go) and locomotion (moving through space and avoiding obstacles). Regardless of the types of visual cues provided, it needs to be able to help us orientate ourselves, thereafter plan the route by making the start and end points clear, help us to monitor our progress as we move towards our destination, and finally recognising the destination once we have reached it. At the end of the day, it is about how we perceive, experience, and use the visual cues in the built space, and think about what to do with it.

While both are wayfinding projects, the big difference between the two projects is that the murals in Kebun Baru were done in a way that considered how persons living with dementia take in visual cues (Information), and our subsequent response to it. This was possible because the project had two persons with the lived experience as co-planners and decision makers. Hence, our sensitivity to having a low threshold of environmental stimulation, potentially being distracted by irrelevant visual cues to problem-solve in a wayfinding task, and our weak ability to process information. These were all taken into consideration for the placement and design of the wayfinding murals in Kebun Baru.

The murals were painted only at key points that will support decision-making and a way to monitor that one is on the right track to home. The colour of each mural is kept to a maximum of three colours because stimulation involving more than one sense can easily overwhelmed our limited attentional functions. This also explains the rational for the comparatively smaller-sized numerals for the block numbers to avoid competing for attention with the murals. Unlike colours, carefully chosen murals that considers the cultural and age factor of a particular neighbourhood add a touch of meaningfulness and familiarity on top of being distinctive and legible.

Anjang: To me, when an entire pillar is painted in a particular colour, I will get slightly overwhelmed. There is also no associative memory for the designs in Nee Soon. When there is too much information, I will not be able to process all the details that are given, and it increases the confusion for me. I really like the ones in Kebun Baru, as the murals are catchy, and reflect culture and reminiscence. There is a lot more character for the murals in Kebun Baru. They make me smile and happy. For me, this is a lot more helpful for persons with dementia, as the murals also bring back fond memories when I see them.

5. As persons living with dementia, do share with us what else we can do to aid in wayfinding for persons living with dementia, especially in our neighbourhoods?

Emily: For me, I would advocate for dementia-sensitive levels of lighting in the neighbourhood and walkways. Good lighting is particularly important for persons living with dementia to minimise anxiety. Poor lighting reduces our ability to make sense of our surroundings and increases the difficulty in navigation. Hence, general lighting in the walkways and void decks, as well as localised lighting in the elevator areas can be further explored to aid in wayfinding.

Anjang: Because of my Lewy body dementia, my visual perception is affected. Do not use dark-coloured tiles and be mindful of the lighting used that it does not cast dark shadows. Dark shadows make me fearful because of the possible illusions that I may have when I see these dark shadows.

6. As self-advocates speaking up for people with dementia, do share with us how can the society be more inclusive for persons living with dementia.

Emily: We need a society that can accept and appreciate people with dementia as individuals who think, behave, and express ourselves differently from societal norms. Despite living with cognitive impairments, we have common needs like everyone – the need to feel accepted and valued as a fellow member of society.

Anjang: Everyone in the community should learn how to recognise and aid persons living with dementia. Do not argue and be patient with us.

There are days when I am overwhelmed by too many stimulations in the community – noise, sights, signs, traffic, crowd, and when this happens, I will get agitated and react unpleasantly. So, to me, dementia awareness is key for any community to become dementia-inclusive. Unfortunately, because of the invisible nature of our condition, you will probably only know that I am living with dementia when I react negatively. On other days when I am not triggered, I am constantly learning to cope and navigate on my own. So, do not take our reactions as something negative. Rather, be patient with us, and understand that our reactions are simply our way of telling you that we need help.

This post was edited from the original post on the Dementia Singapore website.

While it does impact an individual’s productivity, efficiency, and effectiveness as a worker, the skills and experiences that they’ve acquired over the years can compensate for functional deterioration.

The term “workability” refers to the relationship between an individual’s resources and job scope. According to the work ability model by the Finnish Institute of Occupational Health, there are four interrelated tiers: with a base layer consisting of personal health and functional capacities; the next layer which is competence and skills; followed by the third layer which includes personal values, attitudes, and motivation; and the topmost layer is work, referring to work scope, work environment, organisation, and leadership.

Hence, it would be useful for any workplace to consider and decide the kind of work intervention needed to keep their employees living with mild cognitive impairment (MCI) or YOD. Beyond economic benefits, retaining the occupational role is vital to provide a sense of purpose in life and maintaining self-worth.

For this to work, the collaboration between the affected employee and the Human Resource Department is crucial. The employee has to feel safe enough to disclose the level of functional capacity without being discriminated against and judged.

The negative attitudes, which are often based on stereotypes and myths, include people with dementia who are unteachable and burdensome. Such perception can influence management decisions and implicates the employee’s situation.

With an inclusive work culture and willingness to support employees living with dementia to maintain their work skills, this is a feasible practice. It is also important for the government to formulate public health policies conducive to supporting those diagnosed with dementia at the workplace. This is especially necessary with the increasing number of cases of YOD over the years.

This commentary is in response to “Forum: Firms need to understand dementia better as the workforce ages” (The Straits Times, 19 January 2021) by Paul Heng, Management Committee Member at ADA

.

We are proud of Ms Emily Ong, a dementia advocate in Singapore gives her poignant thoughts as someone living with dementia at the recent #ADI2020 Global Conference. Read her speech as she shares about her diagnosis and her hopes for living a normal life with dementia.

I am Emily Ong, a dementia advocate, co-facilitator of ADA (Singapore) Voices for Hope programme, Focus Group member of the Global Dementia Observatory, and a member of Dementia Alliance International. It is a privilege to have been given this opportunity to speak on behalf of my global friends who are living with dementia, an affirmation of ‘Nothing about us, without us.’

My journey with dementia started with the French Toast incident. French Toast is one yummy, yet simple breakfast to prepare. However, that was not the case for me on one fine, usual morning. I suddenly found myself struggling to remember what ingredients I needed, and how to make French Toast. All I knew was that… I needed bread. I had no recollection or impression that I had made it before. It was like my brain had a blackout…. there was nothing to retrieve at all. That was when my elder daughter, a first year medical student back then, saw an urgent need to make an appointment with a neurologist specialised in cognitive neurology.

I was diagnosed with cognitive impairment and young onset dementia at 51 years old. It was the worst nightmare I could have ever imagined. How I wished it was just a horrible dream. There were so many unanswered questions. How was it possible that I had dementia despite being young and healthy? Was I going to be useless and needing to depend on others for everything? Were there other young people like me who have this condition? Would I be able to live life as I always had?

There was no post-diagnostic support system in place, to help me process the rawness and brutality of this diagnosis. There was no one to hold our hands and help us to grieve for the loss of the future we had planned. I felt so alone and terrified, and yet, I had to put up a brave front for my family, that things will be okay. I cried day and night, until I couldn’t cry anymore. All I knew was that…there is only one way in dementia, and that is a downward spiral, with no hope of cure and no way to maintain my independence. I was told of daycare services, where activities like colouring and playing mahjong were provided to stimulate cognition. Memory cafes where people sang familiar songs to relive memories of yesteryear. None of this made me feel empowered, engaged or enabled.

I was at the lowest point in my life, when I found Dementia Alliance International and the ‘Voices for Hope’ programme. For the first time since my diagnosis, I saw hope and the possibility of living life with meaning and purpose again.

At DAI and Voices for Hope, we laugh, we discuss, we talk about how to care for ourselves, we inspire and support each other. In short, we are doing all the normal things in life!

This is what I want to share and encourage others to do: live a normalised life. Yes, it will never be the same again. There will be changes, and you might need to adjust your expectations and make adaptations with whatever capacity you still have but… never, never, never doubt your capacity to live your life to the fullest potential.

However, people living with dementia and their care partners need the support from the wider community to have hope again. Do allow me to share my acronym of HOPE with you:

H for Hear us out – and then tell us what you think of our suggestions. Encourage and support us to have our voice heard. Even though people with dementia may change in the way they think, feel, behave, interact and communicate, this does not mean they don’t have feelings, preferences, opinions or thoughts.

O for Offer support – Why offer not provide? Because assistance is meant to enable us to help ourselves and maintain our functioning. The daily capacity to do things has a direct influence on the independence of people with dementia, and their quality of life. So, let the person living with dementia decide the kinds and level of support needed.

P for Partner with us – let us guide you with our lived expertise in our conditions. Involve people living with dementia in the planning of programmes and activities. This not only empowers these individuals, but also leads to more effective use of resources and better outcomes.

E for Enable us – Focus on our remaining capacity. Minimise and compensate for any functional changes resulting from the disability caused by dementia. People living with dementia need rehabilitation programs, it can be neurological or physical rehabilitation, to help them regain the highest level of function and independence possible, while improving their overall quality of life – physically, emotionally, and socially.

So, please embrace the HOPE acronym so that you can shine hope into the lives of those living and affected by dementia. Thank you.

This post was edited from the original post on the Dementia Singapore website.
