There are numerous programmes and services available in the community, owing to the Integrated Mental Health Network. This network brings together health, social, and community care providers who serve various clients, including persons living with dementia and their care partners.  
 
If you are just starting out on your journey with dementia and don’t quite know where to start, this article is for you. Read on to find out about the various programmes and services available, how they can help you, and requirements you might need to take note of before applying. 

CARE & SUPPORT SERVICES

Care & Support services involve the provision of practical (i.e., support with daily activities, linking clients up to relevant care services), emotional and financial support for persons living with dementia and their caregivers. These services may be rendered at home, a day care centre, or a nursing home for clients who require care and support that cannot be provided at home.

SOCIAL ACTIVITIES/PROGRAMMES

Social activities and programmes create opportunities for interaction with others in the community, which is especially pertinent for persons living with dementia who may become more withdrawn over time as they socialise or communicate less. These activities and programmes are aimed at slowing down the progress of dementia, providing a structured schedule, and improving the mood of persons living with dementia.

MEDICAL AND HEALTHCARE SERVICES

Medical and healthcare services for persons living with dementia may include medical assessments, rehabilitation care, prescription of medications to aid with behaviour, memory and mood, as well as nutritional assessments. These services may be rendered in various settings, including the care recipient’s home, centres, hospitals or nursing homes.

Are you a caregiver of a person living with dementia looking for a community group that you and your loved one can be a part of? The Meeting Centre Support Programme (MCSP), where persons living with dementia and caregivers meet with other families living with dementia once a week in a community setting, may be for you.

What is the Meeting Centre Support Programme?

The Meeting Centre Support Programme (MCSP) is a programme model which aims to support persons living with dementia and their family caregivers in adjusting to the changes that dementia brings. Meeting Centres are started by Dementia Singapore in partnership with community-based service providers such as Active Ageing Centres (AACs), Community Outreach Teams (CREST), faith-based organisations, and grassroots organisations. At Meeting Centres, both persons living with dementia and their family members meet at a community-based location in small groups to take part in two hours of interactive activities together once a week. Meeting Centres are designed to be social clubs where persons living with dementia and caregivers gather to socialise, form new friendships with fellow members, and support one another.

Though activities differ from centre to centre, here are some activities that persons living with dementia and their family members can look forward to:

  • Fun, facilitated group conversations with fellow members (e.g. discussing recent events in the news, reminiscing past experiences)
  • Simple physical exercises
  • Games
  • Craft activities

 

Some Meeting Centres hold monthly caregiver support sessions for caregivers attending the Meeting Centre. During these sessions, caregivers participate in guided discussions about dementia and caregiving. Caregivers can also hear from each other about their experiences and mutually provide and receive peer support.

Aims and Benefits of the MCSP

Through the programme, persons living with dementia and their caregivers can better adjust to the changes they are experiencing with the onset of dementia:

  • Changing physical and cognitive abilities
  • Relationships with healthcare professionals
  • Emotional changes
  • Self-image
  • Future plans
  • Social life and ability to support others
  • Journey of seeking help for themselves

 

Persons living with dementia have experienced the following benefits:

  • A positive effect on behaviour
  • Delay of admission to residential care
  • Positive effect on mood, self-esteem, and sense of belonging

 

Caregivers attending the programme have also:

  • Felt better able to care for their loved ones
  • Had been, on average, able to manage caring for their loved ones at home for longer periods
  • Felt less burdened, while more supported by professional organisations
  • Experienced better mental health and less distress

As a person living with dementia or a care partner of one, feelings of frustration are not uncommon, especially when navigating spaces and systems that are not designed to meet your needs. In times like this, have you ever felt like something has to be said and done so that you, and others like you in the community, do not have to feel this way again? Have you ever wanted to take the lead in making these changes? If you have, you may want to consider becoming a dementia self-advocate.

Self-advocates have been essential in bringing about positive change, especially in the disability movement1. In fact, policies, research studies and various other dementia-related initiatives have benefitted from the involvement of persons living with dementia and their care partners in the development phase. 2

Read on to find out what advocacy is, why it is important, and how you can become one.

What Does Advocacy Mean?

Advocacy is about having a voice. It is about speaking up on what you believe in to achieve personal and collective change. Unsurprisingly, advocacy and empowerment go hand in hand, in that advocates are empowered to influence matters that affect them and take charge of their lives. 

In the context of dementia, advocacy can involve the sharing of stories, raising awareness about the issues faced as a person living with dementia or a care partner, and creating a positive change for the dementia community on a larger scale. Persons living with dementia and care partners involved in such advocacy efforts are often referred to as self-advocates.  

Being a dementia self-advocate is no easy feat. It takes a lot of courage and strength to speak up for yourself especially as a person living with dementia, where the common misconception is that persons living with the condition may not be able to think or decide for themselves. However, with the support of an already existing network of advocates and programmes aimed at empowering persons living with dementia to lead purposeful lives, anyone can become a self-advocate. The work put into becoming one will also be worthwhile as you will be better equipped to represent your community, as well as enable and empower others around you to build a dementia-inclusive society.  

Why Should Persons Living with Dementia and Their Caregivers be Involved in Self-advocacy Efforts?

Research has shown that dementia advocacy offers potential improvements in well-being for those involved, through the activity itself and via extension of the advocate’s social networks.3 While these benefits are notable, it is also important to recognise that: 

  • Persons living with dementia and their caregivers have a right to be involved in decisions that impact their lives.  
  • Persons living with dementia bring with them lived experiences, knowledge and skills that may not be obtainable from organisational stakeholders. Furthermore, dementia manifests itself differently in every person living with dementia. This means that more voices are needed to ensure diverse and accurate representation of dementia in the community.  

What Does Advocacy Look Like in Singapore?

As Singapore works towards the goal of dementia-inclusivity, significant effort has been put into raising public awareness about dementia and making the necessary amendments to services, policies and physical spaces to better meet the needs of persons living with dementia and their care partners. This opens up more space for dementia advocacy, which allows advocates to act as story tellers, co-facilitators, co-creators and active contributors to the conversation surrounding dementia.

Advocacy efforts come in all shapes and sizes. Self-advocates can be involved in as much or as little as they would like in a wide range of activities, including: 

  • Research opportunities 

Involvement in research may include providing advice on research priorities and methodologies, speaking with various stakeholders, being a research participant or working as a research partner on the project. 

  • Participating as a representative on advisory boards or committees 

Advocates can choose to do this on a local and/or global scale.

Providing input and having a say on issues related to the dementia cause 

Emily Ong is a member on the Alzheimer’s Disease International (A.D.I.) Board of Directors for the position of person with dementia. She also serves in the Global Review Panel of the A.D.I. Accreditation Team and is a focus group member of the WHO Global Dementia Observatory Knowledge Exchange.

  • Sharing their stories on various platforms

Storytelling is a powerful way to connect with and inspire one another, which explains why it is an effective tool in advocacy. Advocates may choose to share their stories in various ways such as on their personal social media platforms and in media interviews.

As the primary caregiver to Rodney, her husband living with dementia, Jacinta’s caregiving journey has been filled with ups and downs. But through it all, she continues to do her best to support him in all that he does and empowers him to live his life meaningfully. Theirs is a story of love, courage, and perseverance.

  • Providing feedback on various initiatives, policies and dementia care innovations

The involvement of advocates, especially from the planning stage, is effective in identifying service gaps as well as examining existing policies, services and solutions to ensure that they are meeting the basic and evolving needs of the dementia community.

SBS Transit Find your Way Initiative Dementia Advocates

SBS Transit & “Find Your Way” Initiative

A collaborative effort between SBS Transit and Dementia Singapore, “Find Your Way” aims to help persons with dementia navigate bus interchanges and MRT stations with ease. This wayfinding initiative was inspired by past research studies, which have found reminiscence to be effective in stimulating parts of the brain that deal with long-term memory and cognition.

Dementia self-advocates Anjang Rosli and Emily Ong served as advisors to this initiative.

  • Public education

As more focus has been placed on increasing public knowledge and awareness about dementia, many learning institutions and workplaces have actively sought out experts in the field of dementia to educate students and staff on the topic. Coupled with facts and information, advocates also share their lived experiences and call-to-action with those present.

The Many of Voices of Dementia Advocacy is a short film with seven people living with dementia in different countries, talking about their experiences of self-advocating, their motivation, and top tips. It was produced by Alzheimer’s Society, 3 Nations Dementia Working Group (3NDWG) and Dementia Alliance International (D.A.I.) as part of a project exploring dementia self-advocacy and inclusion.

Voices for Hope

Voices for Hope is an empowerment programme aimed at empowering persons living with dementia and their care partners to embark on their self-advocacy journey. Piloted in 2019, it is a 10-week programme that equips participants with relevant skills and fosters confidence in them to actively share their stories, their needs and views publicly. The goal of Voices for Hope is to change societal attitudes, reduce the stigma of dementia and improve the landscape of how persons living with dementia are perceived and supported.

Who should join

Voices for Hope is best suited for persons who are diagnosed with mild-stage dementia, fairly proficient in communication and articulation, and with stable care arrangements.

The Agency for Integrated Care (AIC) has introduced  the Cognitive Intervention Programme (CIP) — an evidence-based programme designed to support persons diagnosed with mild to moderate dementia. It aims to maintain and improve cognitive and social function through structured multi-component activities. Through these three-hour sessions (across seven weeks, twice per week), participants can enjoy activities such as physical exercises, cognitive training or stimulation, and social engagement.

Benefits of the Cognitive Intervention Programme (CIP)

Participants are engaged through structured, multi-component activities, and have the opportunity to befriend peers through CIP. CIP has a monthly caregiver engagement session to provide resources and knowledge needed to support persons living with dementia at home. 

Potential benefits of CIP:

  • For persons living with dementia
    • Maintain or slow down decline of cognitive function 
    • Maintain and improve functional status, social engagement, mood and overall well-being 

 

  • Alleviating caregiver burden

 

Willie Chiang's Story

After joining the Family of Wisdom, Willie’s caregiver, his sister, noticed that he could adapt to the activities, and understood instructions. Willie also began taking small steps in engaging and communicating with staff and peers in his group. 

Overtime, Willie’s family members and caregiver observed that he enjoyed the activities and could foster friendships with others. At home, he became more active by moving around, watching TV, reading newspapers, and going to the neighbourhood mall for haircuts and handled his basic needs. He was also more cheerful, jovial, and less lethargic. 

Willie’s caregiver also noticed that he could remember his personal particulars, and could manage small sums of money independently.

Willie opted to complete the CIP within six months. 

Participant considerations

Participants diagnosed with mild to moderate dementia are suitable for CIP.

Caregiver accompaniment may be required during sessions for participants who:

  • Are at higher risk of falling in general
  • Need assistance with sit-stand transfers
  • Need assistance with toileting

Who Can Sign Up, and How?

CARA membership is available to all Singapore residents. Members living with dementia will receive a physical CARA card and e-card in the app. Caregivers will only receive the e-card.

Are you a family caregiver looking to enhance your caregiving skills for your loved one living with dementia? The Caregivers Training Grant (CTG) offers an annual subsidy of $200, allowing you to attend approved courses tailored to improve your caregiving skills and knowledge. 

Eligible beneficiaries of the CTG will start off with a $400 grant. Each year, there will be a top up to care recipients’ CTG by $200, and unused monies can be carried forward, with the CTG monies for each care recipients capped at $400.

Given that caregiving often involves multiple caregivers, including family members and migrant domestic workers (MDWs), you may also want to consider encouraging them to sign up for these courses.

Reminiscence Arts for Dementia is a facilitated learning journey which prepares learners to conduct small-group interactive reminiscence arts sessions for persons living with dementia using objects, storytelling, and artwork materials.

This blended course aims to guide learners with theoretical and practical knowledge of preparing individual profiles and build these characteristics into their design, preparation, and implementation of reminiscence group activities. Live demonstration and skills practice are expected in this programme, where the key principles of person-centred approaches are spotlighted, deepened, and integrated in the group learning.

  • Introduces the principles of Person-Centred Care 
  • Learn to select the appropriate communication strategies to create meaningful interactions with persons with dementia 
  • Explains the value of reminiscence in bridging relationships with persons with dementia 
  • Learn the use of artefacts to prompt meaningful conversations

Enabling EDIETM (Educational Dementia Immersive Experience) 

This is an immersive workshop that enables participants to see the world through the eyes of a person living with dementia. Utilising high-quality virtual reality technology, this workshop enhances participants’ knowledge of the impact of dementia and adopts reablement approaches to improve the quality of life for persons living with dementia.

  • Describes dementia from the perspective of a person with dementia
  • Identifies support needs in partnership with persons with dementia and their caregivers

Training for Foreign Domestic Workers

In-person Course

Foreign Domestic Workers (FDWs) Core Module – Essentials of Dementia Care

Foreign domestic workers may find it challenging to cope when persons with dementia  exhibit certain behaviours, such as repetitive questioning, agitation, and anger. They may find it difficult to assist persons with dementia in daily activities such as eating, taking showers, and going to the toilet. As the condition progresses, communication between the person with dementia and the FDW may also decline. 

“Essentials of Dementia Care” is a one-day workshop that offers core information and useful techniques for FWDs to apply when caring for persons with dementia.

This module is conducted in English, Malay, and Burmese.

Course Objectives  

  • Identifies signs and symptoms of dementia
  • Learns the impact of dementia on behaviours of the person
  • Identifies strategies to manage behaviour changes
  • Applies techniques to communicate with the person with dementia effectively

What if we could combine caregiver support activities with their interest to acquire new skills? Dementia Singapore has done just that! The Caregiver Support & Network (CSN) is a refreshing and new programme launched in December 2020 to better support a caregiver’s needs and well-being.

Facilitated by Dementia Singapore’s social workers and volunteers, caregivers are encouraged to take a break from your intensive responsibilities, have some needed respite, and experience activities beyond the regular Caregiver Support Groups. CSN offers a safe space for caregivers to nurture their own interests or learn new skills, while at the same time connecting with like-minded individuals with similar experiences.

Through this programme, Dementia Singapore aims to promote the importance of self-care amongst caregivers and to foster interactions with other caregivers caring for loved ones living with dementia, through hobbies and other interest-based activities such as themed musical performances, learning basic Cantonese, or even learning how to play the guitar. 

The current CSN sessions offered are:

Basic Canto-Lingo

Always wanted to learn Cantonese to converse with your grandparents, or with the elderlies in your community? Caregivers can now finally set aside the time to learn basic conversational Cantonese on a bi-weekly basis, including slang words!

Hatha Yoga

In this type of yoga, participants learn how to move their bodies slowly and deliberately, challenging their strength and flexibility, while at the same time focusing on relaxation and mindfulness. The perfect activity for a stress-free period of respite and self-care.

Knits & Knots

As a beginner’s guide to knitting, this series aims to gather like-minded caregivers with a passion for knitting and creating meaningful pieces, while connecting with other caregivers in a fun-filled environment. The sessions are conducted by mother-daughter trio Katherine Lim, Belinda Seet, and Barbara Seet, graduates from Dementia Singapore’s Voices for Hope programme and dementia self-advocates.

Making Memories

As a beginner’s course to photography skills, participants get to try their hand at shooting basic portrait, landscape, black and white photos and more, using just their smartphones.
The Making Memories group is no longer active.

Relax with Zentangle

Zentangle drawings are a way of drawing abstract patterns that promotes mindfulness and creativity. Let your creative juices flow in Relax with Zentangle, which is facilitated by peer leaders and staff.

Date With Donnie

A live-streamed series featuring our regular volunteer and professional musician, Donnie Chan, as he serenades participants with evergreen rock ballads and greatest hit songs.

Strum & Sing

Designed as a ‘how to’ course, this series brings together amateur string-pickers as they attempt to master the guitar in just ten sessions!

Increasingly, Covid-19 has taught us that getting the right support and mental stimulus is key to fostering a sustainable and happy caregiving community. Numerous reports of caregiver burnout have been seen in Singapore in recent years, exacerbated by the pandemic and social isolation. Dementia caregiving accounts for a large number of such cases due to the unique needs of dementia care and the limited caregiver respite outlets. Caregiver Support & Network is yet another service gap that Dementia Singapore hopes to fill.
